  • All patient rooms are private with room for family overnight stays.
  • Patient to staff ratio is low, providing maximum individualized attention.
  • Dialysis services are conveniently located on the rehab unit floor.
  • Discharge planning is provided to ensure the necessary support system is in place once the patient leaves and to resolve any potential issues.
  • Regular communication is maintained with all referring individuals and outcomes reports are available.

The Rehabilitation Process:

photoThe rehabilitation process begins with a comprehensive assessment of the patient’s functional abilities and goals for recovery. Based on the results of this evaluation, an individualized therapy plan is developed. All therapy plans are customized based on the individual’s existing abilities, tolerance for therapy and desired outcomes and are coordinated by a group of healthcare professionals trained and experienced in rehabilitation medicine. The team meets regularly to discuss each patient’s progress with goals and to make necessary adjustments to the treatment plan.
